3. Completely filled-out Affidavit of Support Form.  One form for each person you are supporting.

4. All other supporting documents
5. NZ Post Courier bag + postage

Affidavit of Support

Clients MUST personally bring all 5 items: 

1. Passport (actual passport)

3. Completely filled-out form:

      Parental Support and Consent Form 
          (Travelling out of  Philippines)

                                OR

     Waiver for Exclusion Ground (WEG)
     (Travelling into Philippines from NZ)

4. All other supporting documents (Ex. Copy of Child's passport)
5. NZ Post Courier bag + postage

Travel of Minors (entry to or exit from Philippines)

Clients MUST personally bring ALL 5 items:

1. Passport (actual passport)
2. Photocopy of Passport (one copy for each document you require)
3. Completed SPA form:
     - request SPA form, and/or details to be 
       recorded in the SPA, from the 
       organization in the Philippines that is 
       requiring the consularised SPA from 
       you 
                      OR

      - download template SPA form from 
     the Philippine Embassy website 
(https://www.philembassy.org.nz/download)
4. All other supporting documents
5. NZ Post Courier bag + postage

Special Power of Attorney (SPA)

Clients MUST personally bring all 5 items: 

1. Passport (actual passport)
